Office Guidelines Regarding COVID-19

Even though the CDC has relaxed its mask requirements, the Oregon Health Authority (OHA) has decided to keep masks in medical settings. Sorry for the confusion, but masks are still required at this time.
As a patient:
Please arrive on time for appointment. The waiting room is open so come on in for your appointment.
Masks are still required in medical settings.
If you have any symptoms of infection (sore throat, cough, fever, shortness of breath, loss of taste) or have been in contact with someone who is sick please call the clinic and reschedule your appointment.
If you or anyone in your household has tested positive to COVID-19 in the past 2 weeks please call the office.
There are no late cancellation fees if you are ill or have possibly been exposed.
If you become sick after being in the office, with COVID-19 or have similar symptoms, you will call and let me know ASAP.
